WARNING: Referee Evals should be filled out by Head Coaches ONLY.  Managers or Asst Coaches may fill them out for their Head Coach but ONLY if the contents are discussed with the Head Coach FIRST.  RIHOA only responds to Head Coaches so make sure it is clearly stated the comments were discussed with the Head Coach before the form was filled out!
NOTE: Ranting is not professional.  Comments such as "inconsistent", "refs not working together", or "not in position" are better wording esp. if specific examples are given.

Empire House (RWB thru Midget):
To get to the EAHC Referee Evaluation Form, use this link EAHC Ref Eval Form .  If this link does not work, go to www.empirehockey.org and click on the Forms Tab at the top of the page, then select "EAHC Hockey Officials Evaluation Form".  Note that this form can be used for all types of games: League, Scrimmage and Local Tournament Games.  All EAHC Teams are required to fill out a Ref Eval for EVERY game (including all RWB Games!).

Travel & Girls Teams:
For local games only: Go to RIHOA Officials Eval Form or go to www.rihoa.com, then select "Officials Evaluation" off the menu on the top of the page.
Out of Town Games: If this becomes an issue, please address your concerns to your WNYAHL Rep (travel@wyha.com) or GLGHL Rep (girls@wyha.com).  They can try to find out if other areas have ref eval forms or ask at the WNY or GLGHL meetings.  For games within NYS, you may fill out the Referee Eval Form for local games but there is no guarantee the local supervisors will be able to pass on your feedback and get you a response. This method is more likely to work if the games occurred in West Section.
Out of Town Tournaments: The Head Coach should address their concerns to the Tournament Director and politely ask if there is a local referee organization with a evaluation form.
